1. Microsoft Word format - This Word document allows the
user to fill out the forms using Word or a compatible word processing
program.
2. PDF format- requires the user to print out the forms
to complete manually. This format requires the Adobe Acrobat Reader
plug-in for your web browser. If you don't have the plug-in installed,

3. Microsoft Excel
format - Two of the forms are available in this format only.
Excel format allows you to complete the form and calculates the
results for you.
